Group Medical Plan

Comprehensive health insurance with medical, mental health and prescription coverage. A high- or low-deductible option is available through PPO Oklahoma Network. You may choose from employee, employee plus child(ren) employee plus spouse or family coverage.

Prescription coverage through participating pharmacies. Prescription co-payment are generic $10, formulary $25 and non-formulary $60.

Group Dental Plan

Provides 100% of reasonable and customary charges, without a deductible, for preventive care that includes two exams per year. After meeting an annual deductible, the plan will cover 80 percent of basic services and 50 percent of major services. In addition, there is a $1,000 lifetime coverage for orthodontics.You may choose from employee, employee plus child(ren), employee plus spouse, or family coverage.

Group Vision Plan

Routine vision care includes coverage for one eye exam per year, frames every two years and lenses or contacts every year. The plan pays 80% coverage up to a $300 per year maximum without a deductible.

You may choose from employee, employee plus child(ren), employee plus spouse, or family coverage.

Term Life Insurance

Core coverage equal to one times annual salary or a minimum of $10,000. Eligible employees are given the option to carry additional life insurance on themselves, spouses and their dependents.

Short Term Disability

Income protection for eligible employees unable to work for health reasons. Pays 50 percent of weekly earnings (not to exceed $1200) after the 14th day through 13 weeks with medical certification.

Long Term Disability

Income protection of 50 percent of monthly salary provided for total and permanent disability following a 90-day waiting period. Employees may elect to purchase additional coverage to raise benefits to 70% of income.

Paid Time Off (PTO)

PTO combines vacation, holidays, sick and personal days into a flexible plan with a range of 23 - 34 days per year as determined by years of service. Coworkers can accumulate up to a maximum of two times their annual amount and may sell back extra days for pay.

Begins accruing immediately and is available for use after 90 days of employment.

Extended Medical Benefit (EMB)

EMB can be used for an employee's own illness and is available after PTO benefits are depleted.

Personal Pension Plan

Company-funded pension plan with financial assistance for your retirement at no cost to you. You are 100 percent vested after five years of qualified vesting service. Hospital makes annual contributions based on the employee's earnings and years of service.

After completion of one year of service with 1000 hours worked.

Matched Savings Plan

Employer matched contribution deposited to your individual Metlife 401(k) account, equal to 50 cents per dollar on the first 1 percent of annual salary and 25 cents per dollar on the next 3 percent of annual salary if the employee contributes at least 4 percent of annual salary.  You are 100 percent vested after three years of qualified vesting service.

Technical and non-technical educational programs are offered to all employees. These programs offer knowledge and skill development in areas ranging from job specific to fundamental business and interpersonal techniques.

Employee Health Service

On-site professional staff providing pre-employment health screens, immunizations, initial emergency or episodic care for work-related injuries/illnesses and counseling/ education. The pre-employment health screen includes a history, physical and drug screen and must be completed three to seven days prior to beginning employment.

Long-Term Care Insurance

Coverage for care in a nursing home, assisted living facility and hospice facility or for community-based services or care in the home. The plan covers employees, spouses, parents, parents-in-law, retirees and their spouses. Premiums are based on age and level of coverage.
